Program of All-Inclusive Care for the Elderly (PACE) helps the participant receive the health care needed. PACE also solves challenges for eligible seniors like transportation to and from medical appointments. Jordan Valley Senior Care (JVSC) is the first PACE program to open in southwest Missouri.
PACE Program
PACE is typically funded by Medicare and Medicaid to deliver necessary care. All your necessary healthcare services may be provided if you are covered by Medicare and/or Medicaid.

Each PACE participant has a team of providers and health navigators who create a custom care plan to help you or your loved one stay healthy.

JVSC Day Center
- The Day Center is open five days a week
- Some medical providers are on site
- Some therapies and treatments are available at the center
- Our staff can help with bathing and personal care
- In many cases, personal care assistance in the home is an essential part of retaining independence at home. Jordan Valley Senior Care PACE offers skilled home health and/or home care for participants who meet the need for these services as determined by the Interdisciplinary Team.

PACE Is All-inclusive Care
- PACE participants receive primary care, necessary referrals for health specialist care and home care among other services, (other than emergency services,) from Jordan Valley Senior Care PACE as the sole service provider.
- Participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
- PACE services are effective the first day of the month following enrollment.

- JVSC does not discriminate in the delivery of PACE services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities or source of payment.
- Appointing a Representative
- You have the right to appoint someone to act on your behalf. You can name a relative, friend, advocate, doctor or anyone else to be your appointed representative. This appointed representative may request a coverage determination on your behalf, as well as file a grievance or appeal.

- Emergency services are covered.
- PACE services are effective the first day of the month following enrollment. You or your loved one can disenroll at any time by providing verbal or written notice to Jordan Valley Senior Care. The disenrollment effective date is the first day of the following month.
- Participants who enroll in a separate Medicare Advantage Plan or Medicare Part D plan while enrolled in PACE will become ineligible for continued enrollment in PACE.
What Does PACE Cost?
PACE is typically funded by Medicare and Medicaid to deliver necessary care. All your necessary health care services may be provided if you are covered by Medicare and/or Medicaid. As a PACE participant, all health care services must be provided or arranged by your PACE team (except for emergency services).
There is no cost to submit an enrollment application. Your application is screened by the JVSC intake staff to verify basic eligibility. We complete assessments to determine a prospective participant’s safety in the community.
The services provided by JVSC are covered, in part or in whole, by Medicare and Medicaid. There are no copays or out-of-pocket expenses for participants who are eligible for both Medicare and Medicaid. A participant will have a fixed monthly premium, or obligation, if:
- Determined by Medicaid to have an income above the protected level; is not eligible for Medicaid.
- Not eligible for Medicare and Medicaid; the participant will pay a premium equal to the Medicare and Medicaid premium.
PACE participants receive all needed healthcare, including primary health care and physician services (other than emergency services) from Jordan Valley Senior Care and may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services. Emergency services are covered.
All healthcare services needed by the participant, and approved by the Interdisciplinary Team, are covered by the PACE Program. JVSC participants Medicare Part D coverage for prescription medication.
