Prepare for Your Visit
We’re excited to meet you and your family. Before you make an appointment, you may have questions for us about insurance, payment and how to connect with other resources.
Make An Appointment
You can schedule a visit by calling us at 417-831-0150. You may also request an appointment online. After you request an appointment, our team will reach out to you by phone to schedule the visit.
Arrive Early to Your Visit
Plan to arrive at least 20 minutes before your first appointment. You will need time to complete paperwork. When you walk through our doors, our team will direct you to the correct waiting area and help you check in. Returning patients should arrive 10 to 15 minutes early to check in.
Needed Documents and Items
Bring these items and documents with you.
- 1 Current government issued photo ID
- 2 Social Security card
- 3 Insurance provider cards and/or MO HealthNet ID cards
- 4A list of all current medications (or the bottles)

Complete Patient Paperwork
When you arrive at our office, you will be asked to fill out paperwork. You can print and complete this paperwork in advance or you may fill it out when you arrive.
