Behavioral Health Services

As part of the Jordan Valley Community Health Center mission, we provide our patients with access to behavioral health services. 

Behavioral Health Consultants are part of the Jordan Valley team and see patients Monday through Friday, 8 a.m. – 5 p.m. Behavioral Health Consultants see patients in Women’s Health, Pediatrics and Family Medicine, once a patient has established care with a Jordan Valley provider. Behavioral Health Consultants are the gatekeepers of Behavioral Health services. Traditional Behavioral Health services include treatment with psychologists and psychiatric consultation. As part of our mission, we partner with organizations such as Burrell Behavioral Health, Pathways Community Health and Preferred Family Healthcare.

The Behavioral Health consult model at Jordan Valley allows for:

  • A functional assessment, given by a Behavioral Health Consultant
  • Followed by a clinically appropriate plan of action which includes an immediate and brief intervention
  • Short term intervention, should the patient need it
  • Referral to a psychologist
  • Referral to additional resources
  • Assessment for psychiatry

Behavioral Health services include:

  • Diagnostic assessments
  • Short-term, targeted interventions for mental health diagnoses & behavioral health changes
  • Help with changing behaviors that may be negatively impacting a patient’s health
  • Behavioral management of chronic pain
  • Stress management skills
  • Help coping with the diagnosis of a chronic disease
  • Skill training to improve parent-child relationships
  • Arrangement of referrals for psychological evaluations, outpatient therapy or other services as needed
