Springfield, MO— Comprehensive care requires integration of services and programs, something medical and dental providers at Jordan Valley Community Health Center understand. For many years, medical and dental providers alike have worked tirelessly to provide a model of integrated care that treats the whole patient. In April 2013, one out of every four patients took advantage of Jordan Valley Community Health Center’s integration of services.  In 2015, one out of every three patients took advantage of both medical and dental services provided through Jordan Valley Community Health Center.

Jordan Valley Community Health Center Sees Improvement for Patients with Hypertension

Springfield, MO— One in four Americans battles hypertension, also known as high blood pressure.  While the exact cause of high blood pressure is not known, there are various risk factors that contribute including: being overweight or obese, drinking too much alcohol, smoking, stress and having a family history of high blood pressure. For patients of Jordan Valley Community Health Center, a diagnosis of hypertension allows for educational opportunities between the patient and their medical team to reach goals that include lower blood pressure.

Since 2013, medical providers at Jordan Valley Community Health Center have viewed improvement in the management of high blood pressure for patients diagnosed with hypertension. Reaching a blood pressure of 140/90 is not an easy task and takes daily consideration of diet and exercise. In 2013, the percentage of patients diagnosed with hypertension who reached or maintained their blood pressure of less than 140/90 included 56.1% of Springfield patients, and 63.5% of Marshfield patients. In 2014, the percentage of patients diagnosed with hypertension who reached or maintained their blood pressure of less than 140/90 included 56.0% of Springfield patients; 69.5% of Marshfield patients; 67.9% of Hollister patients; and 46.4% of Republic patients. In 2015, the percentage of patients diagnosed with hypertension who reached or maintained their blood pressure of less than 140/90 included 56.0% of Springfield patients; 64.7% of Marshfield patients; 65.9% of Hollister patients; 58.2% of Republic patients.

Jordan Valley Community Health Center Moves to Improve Numbers for Diabetes Patients

Springfield, MO—Medical staff at Jordan Valley Community Health Center have worked to develop programs and services to assist patients in better management of chronic conditions. For patients with diabetes, their medical team works hand-in-hand to monitor hemoglobin levels through a test called A1c. The A1c is an important test for diabetics because it displays an average of how well the patient has controlled their blood sugar levels over the last two to three months. The information gathered from the results of an A1c test better aid healthcare providers in adjusting diabetes medications for patients, so that diabetes may be better managed.

 

Since 2013, Jordan Valley Community Health Center has developed a program to deliver better diabetes care to its patients. A1c results are now shared with the patient at the time of their appointment so that education or other services may be provided for the convenience of the patient. With the implementation of this program, providers have seen an improvement in the number of patients controlling their A1c levels.

 

In 2013, the percentage of diabetes patients with A1c levels <8% accounted for 54.7% of diabetes patients in Springfield and 63.5% of diabetes patients in Marshfield. In 2014, the percentage of diabetes patients with A1c levels <8% accounted for 53.9% of diabetes patients in Springfield; 67.6% of diabetes patients in Marshfield; 53.3% of diabetes patients in Hollister; and 50.0% of diabetes patients in Republic. In 2015, the percentage of diabetes patients with A1c levels <8% accounted for 58.6% of diabetes patients in Springfield; 67.2% of diabetes patients in Marshfield; 41.4% of diabetes patients in Hollister; and 33.3% of diabetes patients in Republic.
